Got this from McDonald's. The cheese is literally outside the box. How do you even mess up this bad?

one guy put it in the box and say “hey, you forgot the cheese” to his coworker. So he held the bun open while his coworker tossed a slice like a frisbee from across the kitchen “shazaam” tap it and wrap it up boys

I think part of the reason this happens sometimes is because they don’t build the burger. They build two halves of the burger and finalize it by closing the clamshell.

I can see it happening if they dropped the cheese slice on the dirty floor, leaned over to pick it up and put it on the burger while still leaned over with their head below the cutting board (blind cheese installment).

They actually do this really stupid thing where they assemble half on one side of the box and half on the other side and then just shut the box like that to complete the sandwich. Annoys the hell out of me.
